Also, I was rewatching some DEFCON lectures about pen testing and some of the stuff said doesn't exactly jive with reality, I don't think.

Open and unused USB ports in the open are bad but the security guys that do pen testing go as far as to say they insert USB with payload into rack server and then they've got direct access to the machines they've connected the USB drive w/ payload with but... what if the rack server runs Linux and udev doesn't mount automatically, for instance?

Yoss: there may well be scripts that can test against vulnerabilities, but I doubt they'd be much good if the server doesn't mount the drive. however, what if the usb payload was actually a pi or smaller computer, could it launch attacks via the connection itself?

Yeah, these attacks generally don't rely on the host computer mounting USB storage at all. The device either presents as HID and sends key events -- which almost nothing is configured to ignore -- or does some sort of direct attack on exploitable flaws in the USB driver itself.
